Ontario Long-Term Care Medication Management Initiative

As per the Long-Term Care Medication Management Initiative, pharmacies that provide services to residents of long-term care homes should track all successful and unsuccessful therapy conversions through the use of the 4 PINs in the chart below. This initiative was launched in September 2016 by the Ontario Pharmacists Association, with the goal of improving medication management in long-term care homes and supporting quality of care delivered.

  • This PIN would be used upon prescriber notification back to the LTC pharmacy that the recommended switch to a lower cost therapeutic alternative was accepted by both the resident and the prescriber.

LTC Meds Mngt - Unsuccessful Intervention: Rejected by Resident/POA

  • This PIN would be used if the decision NOT to switch to the lower cost therapeutic alternative was made solely by the resident.

Example:

  • A personal preference of the resident to remain on current therapy.

LTC Meds Mngt - Unsuccessful Intervention: Rejected by Prescriber (non-support of switch)

  • This PIN would be utilized when the prescriber rejects the recommendation based on general reasons that are not patient-specific.

Examples:

  1. Prescriptive autonomy – the prescriber generally does not want his/her prescriptive decisions challenged.
  2. Dislike of the alternative agent – the prescriber does not support the use of the recommended alternative(s) for ANY resident.
  3. Lack of access to the complete medication history – the prescriber is unsure if the alternative agent had been previously tried.
  4. No reason given – the prescriber simply responded “no” to the recommendation.

LTC Meds Mngt - Unsuccessful Intervention: Rejected by Prescriber (resident-specific)

  • This PIN would be applied when there is a patient-specific reason why the prescriber is unwilling to switch therapy.

Examples:

  1. Previous adverse event with this resident.
  2. Previous lack of efficacy with this resident.
  3. Other patient-specific safety concerns, such as increased resident morbidity.

Submitting a Claim Using the PINs

  1. Create a new prescription for the patient using the following details:
    • Patient: Patient Name
    • Drug: Based on the scenario, one of the PINs from the chart above.
    • Doctor: Pharmacist handling the therapy conversion
    • Qty Auth: 1
    • Qty: 1
    • SIG: INTERVENTION ASSESSED BY: <Pharmacist Name>
    • Days Supl: 1
